Financial Assistance/Pricing Guide
At SMH, we understand that medical emergencies, unexpected medical procedures and unplanned medical expenses can present financial difficulties for families, especially in instances where the patient is uninsured or unemployed.
SMH does have a Financial Assistance program and you may be eligible.
Below you will find information that is very important to help you determine your eligibility for discounted care.

Required Information for Financial Assistance Program
Patients must call and establish an appointment to apply for Financial Assistance. The following documentation is required:
- Income verification for the last 30 days for each applicant and their spouse
- Your last checking or savings account statement.
- Copies of all monthly bills including: credit card balances, revolving monthly accounts utilities (gas, electric, phone, water, trash, cable, cell phone)
- Rent verification (rent receipt or copy of lease)
- Last income tax return
- Payment book or monthly statement for any outstanding loan
- Any monthly insurance premium (health, life, auto, home)
- Pharmacy printout of last 30 days prescription drugs
- Copy of any retirement fund, IRA, 401-K or pension benefits
All patients will remain financially responsible for their balances until a financial aid application is completed and processed. Be advised that it may take up to two business weeks to process your application in order to deem it complete.
